Medicaid Services
Glenwood Private Homecare is proud to provide compassionate, dependable in-home care to eligible Medicaid members through Georgia’s Community Care Services Program (CCSP) and Service Options Using Resources in a Community Environment (SOURCE) programs.
Our goal is to help seniors and individuals with disabilities remain safe, independent, and comfortable in their own homes while receiving the support they need.
Community Care Services Program (CCSP)
The CCSP program helps eligible individuals who may otherwise require nursing home-level care receive support while remaining in their own homes or community setting.
Glenwood Private Homecare provides personalized in-home support based on each individual's approved care plan and needs.
The SOURCE program helps eligible Medicaid members with chronic health conditions and functional limitations remain in the community by coordinating healthcare and long-term support services.
Through SOURCE, Glenwood Private Homecare provides dependable in-home assistance designed to support each client's health, independence, and quality of life.
Our Care May Include:
Personal care assistance
Assistance with activities of daily living
Respite and caregiver support
Assistance with maintaining a safe home environment
Companionship and supportive care
Other approved services based on the client's Medicaid program and individual care plan
